The federal law, “The Family Educational Rights and Privacy Act,” gives guardians the ability to do each of the following;

1. Inspect and review student’s education records.
2. Request the amendment of the education records to ensure they are not inaccurate, misleading, or otherwise in violation of the student’s privacy or other rights.
3. Consent to the disclosure of personally identifiable information from education records as specified by law except to the extent that the applicable state or federal law authorizes disclosure without consent.
4. File with the U.S. Dept of Ed, a complaint regarding alleged failures.
5. Obtain a copy of district policy with regards to student education records.

If, at any time, you would like to review your child’s educational records, please contact the office.

When your child enters Melrose School, you will be asked to complete a registration form.  It is important that the form be completed fully since it provides the school with emergency dismissal information and provides directions to follow if an accident should occur, and you cannot be contacted.  Each year, you will be asked to update this information.  Kindergartners, first graders, and others entering an Oregon School for the first time must show proof of immunization and birth certificate or other proof or legal document indicating date of birth.
